At the bottom left of the document, the signatures of Charles Cochon de Lapparent and Philippe-Antoine Merlin de Douai.


Possibly, but partly erased: Collot d’Herbois (see photo 10, same line as Merlin de Douai).

The document also bears, among the members of the Health Commission, the signature of Antoine Dubois, the famous surgeon and consultant to the Emperor.
As well as that of Claude Louis Berthollet, the famous chemist.

Document on laid and watermarked paper, measuring 23.5 x 36 cm, on Republic letterhead.
It gives the citizen’s full name, age, place of birth, parents’ occupation, previous positions held, record of service, and notes and observations.



The upper part is frayed and soiled, with a tear in the upper right corner at the word “Liberty,” and two or three tears on the right side along the edge.
